The release of estradiol from ESTRING was demonstrated in a Phase II study of 222 postmenopausal women who inserted up to four rings consecutively at three-month intervals. For oestrogen products for vaginal application of which the systemic exposure to oestrogen remains within the normal postmenopausal range (ESTRING vaginal delivery system), it is not recommended to add a progestagen. Estrogens used in therapeutics are well absorbed through the skin, mucous membranes, and the gastrointestinal (GI) tract. The vaginal delivery of estrogens circumvents first-pass metabolism. HRT use does not improve cognitive function. There is some evidence of increased risk of probable dementia in women who start using continuous combined or oestrogen-only HRT after the age of 65.The standard way to use the ring is you leave it in for 21 days, then remove it and have a 7-day ring-free break. You're protected against pregnancy during the ring-free break. After the 7-day break you then put a new ring in for another 21 days. Estring is an off-white, soft, flexible ring with a center that contains 2 mg of estradiol. The ring releases estradiol into the vagina in a consistent, stable manner for 90 days. It should be removed after 90 days of continuous use. If continued therapy is required, the ring should be replaced.
